In connection with this application for employment, you understand that any offer of employment is contingent upon the following:
- Satisfactory post offer, pre-employment drug screen and physical that will be set up with one of our company physicians and paid for by Hillside.
- Proof of authorization to work in the United States
- Satisfactory post offer, pre-employment driving record check (only for applicants/employees who will be driving company vehicles.)
- Satisfactory post offer, pre-employment criminal background check (only for applicants/employees who will be doing work for clients off site.)

I understand that a background driving check, consumer credit report and/or a criminal records check may be necessary prior to my employment. If such reports are required, I understand that, in compliance with federal law, Hillside will provide me with a written notice regarding the use of these reports and will also obtain separate written authorization from me to consent to these reports. I also understand that a poor driving record, credit history or conviction will not automatically result in disqualification.”
